If you are considering making a complaint about a trade union we recommend that you first consider the guidance note on making a complaint by clicking in the appropriate box below. We also recommend that, before completing the registration of complaint form, you contact the Office to discuss the matter first. Complaints form: if you want to view the form or print it in order to complete it by hand, click on the pdf icon pdf. If you want to complete the form on screen you must first download it to your computer. You can do this by right-clicking on the blue icon rtf, saving the file in a suitable directory and opening it. Complete the form on-screen, then print it out, sign it and post to the Certification Officer with any relevant enclosures.

IMPORTANT MESSAGE REGARDING DELIVERY OF MAIL TO THE CERTIFICATION OFFICE

The Certification Office is currently experiencing difficulties with the delivery of Royal Mail post.  Royal Mail are aware of the problem and are working to rectify it.

In order to guarantee that any complaint forms reaches the office quickly you are strongly advised to deliver by hand, fax it or e-mail it.  This is particularly important when submitting complaints that have to be submitted within strict time limits.  The Certification Officer has no discretion to vary these limits.
